RESUMEN

Purpose: The aim of this study was to detect infections requiring hospitalization in patients with ANCA-associated vasculitis (AAV).Materials and Methods: This is a single-center, retrospective study conducted in Turkish patients with AAV. Infection episodes requiring hospitalization, reproducing pathogens, laboratory findings, immunosuppressive treatments given for the treatment of vasculitis, and the relationship with the infection were evaluated.Results: Seventy-four patients diagnosed with AAV were included in the study. Hospitalization due to infection was observed in 36 of the patients. The coexistence of diabetes mellitus (DM) was found to be significantly higher in the infected patient group. Cyclophosphamide (CYC) treatment found to increase risk of infection. More than 80% of the infected patient group presented with renal involvement (80.6%). A total of 68 infectious episodes were seen in 36 patients. The most common involvement of infection was the respiratory tract with a rate of 70.6%. Gram-negative bacteria were the most common pathogen, especially Pseudomonas aeruginosa. With the effect of the pandemic, SARS-CoV-2 has come to the fore among viral infections. Aspergillosis was the most frequently detected among fungal infections. Besides, aspergillosis was the cause of 85.7% (6 episodes) of fungal infections. Lymphopenia was observed in 76.5% of the infection episodes. 57.4% of infections developed in the first year of the induction therapy. The most frequently used immunosuppressive therapy for the treatment of vasculitis in infectious episodes was CYC (41.2%).Conclusion: Managing infections during the vasculitis treatment is crucially important. Lymphopenia, kidney involvement, DM and immunosuppressive therapy are factors that increase the risk of infection. Clinicians should take preventive measure especially for respiratory tract infections and gram-negative bacteria as pathogens.

Introduction: This study was conducted to examine the relationship between teachers' level of conscious mindfulness and level of coronaphobia. Material(s) and Method(s): This research was made in a descriptive type and the universe of the research;It consisted of teachers working in Erzurum city center. The sample of the study consisted of 450 teachers reached by the Easy Sampling method. Personal information collection form, conscious mindfulness scale, and Covid-19 fear scale were used to collect data. In the analysis of the data collected in the research;percentage, mean, standard deviation, ANOVA, Man-Whitney U, and correlation analyzes were applied and these analyzes were performed using the SPSS-20 (Statistical Package for the Social Science-20) package program. Finding(s): In this study;A significant opposite relationship was found between teachers' mindfulness levels and coronaphobia levels. (r=-.219,p<.01). As a result of the study, the coronaphobia levels of female teachers were found to be significantly higher than male teachers (U=16102,500, p<.05). A positive correlation was found between teachers' coronaphobia levels during the pandemic and their online shopping for food (r=.198,p=.001) and their shopping for clothing (r=.131,p=0.024). Conclusion(s): As a result of this study, it was determined that the level of coronaphobia decreased as the level of conscious mindfulness of teachers increased. In addition, female teachers were found to have higher coronaphobia levels. It was determined that as the conscious mindfulness level of teachers decreased, there was an increase in clothing and food shopping expenditures. RESUMEN

Aim: Varicella zoster virus (VZV) is a double stranded DNA virus from herpes virus family. Only infection source is human and it is very contagious. With this case we want to mention about atypical presentation of zona and it should be kept in mind as differential diagnosis for atypically located vesicular lesions. Case: 50 years old healthcare worker female patient presented to emergency service with parestesia and coldness of one leg followed by vesicular lesions. Predisposing factors are chronic disease, immunocompromising disease, age more than 60 but for our patient no predisposing factors observed. The COVID Pandemic has brought with it other additional health problems.

OBJECTIVE: Although many studies reported prognostic factors proceeding to severity of COVID-19 patients, in none of the article a prediction scoring model has been proposed. In this article a new prediction tool is presented in combination of Turkish experience during pandemic. MATERIALS AND METHODS: Laboratory and clinical data of 397 over 798 confirmed COVID-19 patients from Gülhane Training and Research Hospital electronic medical record system were included into this retrospective cohort study between the dates of 23 March to 18 May 2020. Patient demographics, peripheral venous blood parameters, symptoms at admission, in hospital mortality data were collected. Non-survivor and survivor patients were compared to find out a prediction scoring model for mortality. RESULTS: There was 34 [8.56% (95% CI:0.06-0.11)] mortality during study period. Mean age of patients was 57.1±16.7 years. Older age, comorbid diseases, symptoms, such as fever, dyspnea, fatigue and gastrointestinal and WBC, neutrophil, lymphocyte count, C-reactive protein, neutrophil-to-lymphocyte ratio of patients in non-survivors were significantly higher. Univariate analysis demonstrated that OR for prognostic nutritional index (PNI) tertile 1 was 18.57 (95% CI: 4.39-78.65, p<0.05) compared to tertile 2. Performance statistics of prediction scoring method showed 98% positive predictive value for criteria 1. CONCLUSIONS: It is crucial to constitute prognostic clinical and laboratory parameters for faster delineation of patients who are prone to worse prognosis. Suggested prediction scoring method may guide healthcare professional to discriminate severe COVID-19 patients and provide prompt intensive therapies which is highly important due to rapid progression leading to mortality.
